In partnership with Fraser Valley Health Care Foundation, the Abbotsford Division of Family Practice is proud to present Health Talks: Demystifying Health Care for Seniors — a free, community-based health education series designed to bring accessible, culturally relevant health information directly to our neighborhoods.
These sessions aim to empower individuals with the knowledge and resources needed to navigate the health-care system, improve overall well-being, and connect with local supports. Join us to learn, ask questions, and be part of a healthier, more informed community.
Highlights:
– Learn how to navigate local health-care services and resources
– Inclusive, welcoming environment with a focus on cultural safety and accessibility
– Engaging, expert-led health talks on important community health topics that support healthier individuals and communities
– Light breakfast and refreshments are provided
Health Talk topics include:
- How to discuss transitions into long-term care with family members
- Understanding the MOST document and how to have meaningful conversations with your GP
- An overview of end-of-life and palliative care
- Managing medications during transitions in care
- Choosing a temporary substitute decision maker and power of attorney, including when, why, and how to plan ahead
